Foster-With-Intent-To-Adopt (FWITA)/Adoption Application Or Foster-Only Application

NOTE: IF YOU HAVE LARGE DIRT AREAS OR ANY OF THE FOLLOWING PLANTS IN YOUR BACK YARD, YOU MUST REMEDY THE SITUATION. IF YOU DO NOT AGREE TO DO SO, STOP HERE, AS WE WILL NOT APPROVE YOU AS A POTENTIAL ADOPTER:

Oleanders, Sago Palm, Lantana, Cacti, Aloe and other spiny plants

REMOVAL OR FENCING OF THE ABOVE PLANTS AND COVERING DIRT AREAS WITH GRAVEL AND/OR GRASS ARE REQUIRED FOR THE SAFETY OF A RESCUED DOG.

Thank you for taking the time to fill out this application. We realize it is lengthy, but please fill out this form completely, as it will speed the process. Include area codes with all phone numbers, and please include your e-mail address. A representative from our Home Evaluation Team will contact you as soon as possible after we receive your application and the application fee.. This is a combined application for families who desire to serve as foster-only homes or who want to be considered as a Foster-With-Intent-To-Adopt (FWITA). Please answer the questions in this application with your ultimate goal in mind, whether it is to foster or to adopt a Rescued Golden. If a question does not apply to you, please write N/A in the response area.

The minimal qualifications for adoption considerations are:

  •   NO OUT OF STATE ADOPTIONS.

  •  Dogs will not be “Outside” dogs and there must be minimal exposed dirt on the property due to the high risk of Valley Fever.

  •  No accessible toxic or dangerous plants in the back yard i.e: oleander, cacti, lantana, sago palm, etc. Property has to be monitored for and precautions taken for poisonous snakes, poisonous toads, poisonous scorpions and spiders, etc.

  •  If requesting a dog under 5, fence must be at least 6 feet high and gates must be latched/lockable.

  •  Well kept, hazard-free environment safe for a Golden Retriever.
There is a nonrefundable $25.00 application fee required to submit an adoption application for processing. An application for adoption is not meant for any particular dog, as dogs are placed after review of all applications and home evaluations to make sure that the best suited home for the dog is selected. Upon submission of the adoption application, you will be taken to a payment page where you can pay the $25.00 fee via credit card (will actually cost you $25.75 by PayPal) or you can mail a check in the amount of $25.00 to:

Arizona Golden Rescue
5350 W. Bell Rd., Ste. C122-158
Glendale, AZ 85308.

Please note:

If your residence is a rental property, AGR requires you to get permission from your landlord in writing to have a Golden Retriever or Golden-Mix at your residence. We must receive this form prior to a home visit being scheduled. You should also keep a copy for your records. If your lease specifies that you can have a large dog at your residence then you may send us a copy of that instead.

Landlord Consent Form

The consent form can be faxed to our toll free fax number 877-750-8168 or you can mail it along with your $25.00 adoption application fee.

If we do not receive the application fee and/or the Landlord Consent Form if applicable within 30 days of the submission of the application, we will delete the application from our database and you will have to fill out another application form if you decide to reapply after 30 days. Credit Card (PayPal) payments will begin the process that day and the Home Evaluation Coordinator will acknowledge your application and assign a home evaluator who will be in contact with you to set an appointment for your home visit.
Check payments will start the process as soon as we receive them in the mail.
We will not schedule the mandatory home evaluation until we receive payment and the Landlord Consent Form if you live in a rental property.
